As a patient at Atrius Health, we feel responsible for your care at all times and in all places. Therefore, we offer an extensive range of services to you, all connected to your electronic medical record and with strong communication among your providers. Our medical practices include primary care, more than 50 specialties, imaging, lab, and pharmacy. When our offices are closed, we offer all of our patients a convenient telephone service, where our nurse practitioners, physician assistants and nurses can give you medical advice, make arrangements for urgent visits to your doctor’s office the next day, write prescriptions, or send you to the appropriate emergency department if needed.   

Additionally, we have created strong relationships with renowned hospitals, specialists in other practices, and other care providers. We work to know when you are receiving care at a hospital or other facility so that we can coordinate the care you are receiving. Given the size of our practice, we are able to form highly collaborative relationships that help make your transitions between care settings as easy as possible.

Atrius Health works closely and collaboratively with a group of preferred hospitals with whom we’ve established electronic communications and special programs to ensure that we stay connected with the care you receive in the hospital and are helping to ensure you have the best possible treatment plan post-discharge.

Atrius Health has a selected group skilled nursing facilities (SNFs) where Atrius Health medical staff or others working to our protocols provide care onsite at the facility in order to minimize the length of stay and to ensure transitions are smooth. Our collaboration with these SNFs has led to shorter lengths of stay and lower readmissions to the hospital, so you can return home as soon as possible.
