Register as a new patient

Please call our Central Patient Registration team at 1-800-249-1767 to register as a patient. They can confirm that we take your insurance, so be sure to have your insurance information ready when you call! They will ensure your personal information is entered into our system and help you schedule your first appointment if you have not already done so. They can also provide you with information to sign up for MyHealth Online, our secure patient portal.
