Home-Based Primary Care at Atrius Health

At Atrius Health, our goal is to provide patients with the right care, at the right time and in the right place. That’s why we developed a specialized Home-Based Primary Care program.

Our Home-Based Primary Care team is made up of health care providers specializing in the care of people the older adult. They know that patients aged 75+ often have unique healthcare issues and may require a more attentive, responsive healthcare experience. They also know that making the trip to a healthcare provider’s office is not always possible.

Who can join the program?

This program is being offered to eligible patients aged 75+, based on their healthcare needs, who would benefit from the dedicated care of a provider with expertise in the unique needs of older patients. Please note the Home-Based Primary Care program is currently available only to eligible patients in around the Plymouth area. We hope to expand the program in the future.

How does the program work?

Patients who are invited to join the Home-Based Primary Care Program will move from their current Atrius Health primary care provider, to a care team that includes specialized advanced practice clinicians, a nurse, and other supportive professionals who specialize in the care of older adults. Medical visits will occur where it is most convenient for patients – at home.

Karri received her BSN from UMASS Dartmouth and her master’s in nursing as a family nurse practitioner from the Massachusetts College of Pharmacy and Health Sciences (MCPHS). She has over 30 years of experience working in a home care setting and her clinical interests include geriatrics, palliative care, and dementia. In her spare time, she enjoys gardening, baking, and spending days with her family at the beach.

How did you choose me for this program?

What will be different about how I receive care in this program?

You will have a new Atrius Health PCP and experience specialized primary care provided by a healthcare team that is specially trained to care for older adults. You will be able to spend more time with your providers and have your primary care visits at home. You will also have a new phone number to call when you are ill or need to contact your Atrius Health provider’s office: 774-608-3720.

What do I have to do to join the Home-Based Primary Care program?

Patients are invited to be in this program, based on their healthcare needs.  If you are contacted about this program and would like to enroll, just call the Home-Based Primary Care team at 774-608-3720 and let us know. We will take care of the enrollment process at Atrius Health and ensure all your medical records are shared with your new healthcare team. If you have Medicare Advantage, call your insurance company and let them know you want to switch primary care providers. We’ll make sure you have all the info you need to make this call.

Where is the program located?

Patients are seen at home for all primary care needs, including annual physicals. If you see an Atrius Health specialist, you will continue to go to the specialist’s office.

Where do I get lab work done?

Lab work can be completed at any Atrius Health primary care office with a laboratory.  If you can’t make it into the office, alternate arrangements for lab work may be available, including home test kits for some labs, or a home visit from a phlebotomist.

Who do I call for my meds and refills?

You will call your new primary care provider (PCP) in the Home-Based Primary Care Program at 774-608-3720.

Will I receive vaccinations at home?

Possibly. If you are receiving care at home and you need a vaccine that Atrius Health regularly administers, you will likely receive the vaccination at home. For vaccines that Atrius Health does not typically administer, we will help you find the most appropriate place to receive the vaccine.

Can I still see my specialist(s)

Yes, you will continue seeing your current specialist(s) and have access to other Atrius Health specialists as needed.

Who do I call if I have a problem?

You will call the Home-Based Primary Care team at the direct line: 774-608-3720.

How often will I see my provider?

Care will be provided as often as clinically necessary, for both preventative care and when you are sick.

What are the hours of the program?

Your provider will be available to see you during regular business hours.  Medical assistance is available by telephone 24/7, with access to our after-hours medical staff.

Will I have the same provider all the time or will I be seeing different people?

Our Home-Based Primary Care providers and staff work together as a team so you may be cared for by more than one provider. However, you will have a PCP of record. We strive to make your experience personal and individualized to your situation and needs.

Will I have a doctor as my PCP?

No, your primary care provider will be an advanced practice clinician (APC).

What if I do not like this program? Can I switch back to my PCP?

We’re pretty sure you are going to love it!  But participation is voluntary, and if you decide the program is not right for you, we will help you transition care to another primary care provider.

What does this program cost?

There are no additional costs for this program and costs will be the same as a visit with your current primary care provider. Some services may require co-pays or fall under your insurance deductible, just like regular visits to any of your Atrius Health providers.

Will my insurance accept this?

Yes, nothing will change about the way your insurance works. But you will need to contact your insurance company to tell them about your new PCP. We can help with that!
