Anticoagulation Management Service at Atrius Health

The Atrius Health Anticoagulation Management Service (AMS) is led by specially trained registered nurses and clinical pharmacists dedicated to supporting safe and effective anticoagulation therapy, with a primary focus on patients receiving warfarin. We work closely with internal medicine, family medicine, and specialty providers to deliver coordinated, high-quality care.

AMS services begin with a referral from your clinician after you have been prescribed warfarin. Once you are enrolled in AMS, our nurses and pharmacists provide comprehensive anticoagulation education, manage therapy to meet your clinical goals, order and interpret laboratory testing, and offer guidance to help safely manage your anticoagulation around procedures or surgeries.

We also support primary care clinicians and specialists in overseeing patients receiving direct oral anticoagulants (DOACs), including apixaban, rivaroxaban, edoxaban, and dabigatran. We provide written educational materials for patients and clinical guidance and consultation for providers to help ensure safe and effective therapy.
