General Records

As a new patient, we will need you to authorize release of your medical records to our practice. Please see guidance on the Compass Medical website for options to electronically transfer your medical records to our practice. Once you have obtained your medical record, please use one of the following methods to transfer your record to us:

• Upload it via our secure MyHealth patient portal (must be registered as an Atrius Health patient). Download Instructions (PDF) 
• Fax it to our confidential line at 617-421-3326

Imaging Records

It will be very helpful to your provider now at Atrius Health to have access to your past imaging studies, particularly if you are in active care for a condition which is being monitored with ongoing imaging. Please note imaging studies (x-rays, ultrasounds, MRIs, CT, as well as all breast imaging) are not part of the general medical record release process noted above. Imaging records need to be separately requested from the facility where those imaging studies were completed.

If you had images taken as a Compass patient at one of their facilities in the past 12 months, they used a radiology partner called Jefferson Radiology for most on-site imaging. Please see the Jefferson Radiology release forms for different types of imaging below. Please complete the appropriate release form and email the authorization form to Atrius Health at imagerequest@atriushealth.org so that our practice can be authorized to access your prior imaging studies taken within the past 12 months with Jefferson Radiology.

If your imaging studies were taken at another facility (such as a local hospital), you can contact those facilities to request that imaging studies are released to you to then share with your Atrius Health provider.
