The Mission of Patient and Family Advisory Councils

Atrius Health’s Patient and Family Advisory Councils consist of dedicated groups of patients, family members and pediatric parents who provide consultation and feedback on a variety of topics that impact the delivery of care and the overall patient experience with our practice. We view these Councils as an essential advisory resource that ensures the voice of the consumer is considered in the design, delivery and communication about health care services at Atrius Health. We recognize that our patients, family members and pediatric parents are one of the best resources we have to understand where and how we can make improvements.

At Atrius Health, our vision is to transform care to improve lives. We strive to partner with our patients and communities to make it easier to be healthy. With that goal in mind, we have a Patient & Family Advisory Council (PFAC) comprised of members who share their input and ideas about we can improve patient care, service, and safety at Atrius Health.

We are actively recruiting Atrius Health patients ages 18 and older and parents of pediatric patients to participate on the PFAC.

  • The PFAC meets for 75 minutes in the evening, five to six times a year, virtually over Zoom or Teams.
  • A stipend is provided as a token of appreciation after each meeting.
  • Steps to being considered include completion of a membership application and a brief telephone call with a member of our PFAC team. All your information will be treated confidentially.

We understand that each person's needs and goals are distinct, especially within our LGBTQIA+ community. In a respectful and safe environment, we aim to foster trust and build meaningful relationships through our patient-centered approach. Our specialized LGBTQIA+ clinical teams, who have experience and specific training in meeting the unique healthcare needs of LGBTQIA+ patients, are committed to staying updated on the latest advancements in LGBTQIA+ healthcare. We provide the support, knowledge, and resources our patients need to make informed decisions about their health.

We are actively recruiting Atrius Health patients ages 18 and older to participate on our LGBTQIA+ Patient Advisory Council (PAC) that provides a forum for patients to offer feedback and helps us ensure we are creating an exceptional experience for LGBTQIA+ patients at our practice.

  • The PAC will meet for 90 minutes in the evening, once a month in 2024, virtually over Zoom or Teams.
  • A stipend will be provided as a token of appreciation after each meeting.

Fallon Health – Atrius Health Care Collaborative Patient & Family Advisory Council (PFAC)

FALLON HEALTH – ATRIUS HEALTH CARE COLLABORATIVE PATIENT & FAMILY ADVISORY COUNCIL (PFAC) INFORMATION & APPLICATION

Patients ages 18 and older and parents of pediatric patients with Fallon MassHealth insurance.

The Fallon Health-Atrius Health Care Collaborative (FACC) is a MassHealth Accountable Care Organization (ACO) Health Plan. An ACO is a group of doctors, hospitals and other health care providers that work together with the goals of delivering better care to members, improving the population’s health, and controlling costs.

FACC includes a network of Primary Care Providers, specialists, hospitals, mental health providers, and Community Partners. All Atrius Health Primary Care Providers and specialists are part of FACC. Primary Care Providers can only be part of one MassHealth ACO.

Advisors on our small group council share their perspectives and ideas about how we can improve experiences as both an Atrius Health patient and Fallon health plan member. The council plays a key role in helping both Fallon Health and Atrius Health understand what we do well and where we can do better in supporting patients and their family.

  • The PFAC meets one hour in the evening, four times a year, virtually over Zoom or Teams. For those who do not have access to a cell phone, computer or iPad, we provide telephone access to meetings.
  • A stipend is provided as a token of appreciation after each meeting.
  • Steps to being considered include a brief telephone call with a member of our PFAC team. All your information will be treated confidentially.

About Patient and Family Advisory Council Meetings

Who: Patient and Family Advisory Councils typically consist of 8-10 members. Any patient ages 18 or older, family member of an adult patient or parent of a pediatric patient can apply. It is very important for the group to represent the diverse patient population we serve to help us respond to differing health care needs, and we welcome applicants from different age groups, races, ethnicities, gender identities and sexual orientations.

Where: Usually our Advisory Councils meet in the early evening, virtually using Zoom or Teams. For those who do not have access to a cell phone, computer or iPad, we provide telephone access to meetings.

When: Frequency of meetings varies, from monthly to semi-monthly to quarterly, depending on the Council.

Responsibilities of Patient & Family Advisory Council Members and Applicant Considerations

We are looking for representatives who can:

  • Share experiences about receiving care;
  • Partner with and advocate for the diverse needs of our patients; 
  • Listen to and respect multiple points of view; 
  • Be comfortable speaking in a group and actively engage in process improvement;
  • Share experiences in ways that others can learn from; 
  • Commit to the vision and goals of the Advisory Council and enthusiastically support the overall mission of Atrius Health. 
  • Attend most meetings with a minimum term of one year.

How to Apply

    By phone: Complete the application over the phone with the assistance of a Patient Relations representative by calling 617-559-8440.

  3. Please Note: We have a limited number of spots open, and if you do not hear from us, we will be sure to keep your application on file for when spots become available. After we receive your application and confirm there is still room on the Council, we will reach out to conduct a brief telephone interview.  This is to determine if the Council is a good fit for the applicant and confirm if the applicant can accommodate the time commitments, etc. All your information will be treated confidentially.
