The Harvard Vanguard Sudbury practice cares for residents of Sudbury, Weston, Waltham, and other nearby cities and towns and is conveniently located in the Emerson Medical Building at Chiswick Park on Route 20. The focus of the Sudbury practice is providing exceptional adult primary care.
Our preferred hospital partners include Emerson Hospital, Lahey Hospital & Medical Center, and Beth Israel Deaconess Medical Center.

Our building is open from 8:00am – 5:00pm during the week.

Referrals at Sudbury
Some types of health insurance may require a referral be approved by your primary care provider before consulting a specialist. Your primary care provider will help you in selecting the appropriate specialist for your needs and the patient services/referral coordinator will complete the necessary forms and fax the approved referrals directly to the specialist. The Patient Service Department's telephone is 978-287-9351. If your insurance requires a referral from your PCP, please have this approved before your specialty appointment.
24/7 Medical Advice
Medical advice is available to patients by telephone 24 hours a day, 7 days a week. If you need medical advice after office hours, please call your doctor's office (unless it is a life-threatening emergency, in which case you should call 911). Your call will automatically be routed to our after-hours triage service staffed by nurse practitioners and physician assistants with access to your electronic medical record. Sudbury patients have access to adult and pediatric weekend and holiday urgent care at the Concord practice. Unless it is a life-threatening emergency (in which case you should call 911), you can use our weekend and holiday urgent care hours for most health-related situations.
